Procedures To Follow
 

 
3.  Upon successful completion of the training and receipt of a firearm training certificate from the training entity,
     the applicant must complete a "CCW APPLICATION FORM.". The form should be made available to you
     where you obtained your weapons training or online at the link below.  Application forms and pamphlets are
     also available in the lobby of the Medina County Sheriff's Office, Monday through Friday 7:30 a.m. to 6:00 p.m.
     at the reception desk.  The Medina County Sheriff's Office will process applications by APPOINTMENT ONLY,
     at the Medina County Sheriff's Office, 555 Independence Drive, Medina, Ohio 44256.   Those wishing to
     make an appointment to process their application may contact the CCW/WebCheck Office at 330-764-3629.

 
5.  The applicant must have the following items when coming to the Medina County Sheriff's Office by appointment to
     process their application for a Carrying Concealed Weapons Permit:

      a) Completed application form
      b) Ohio Concealed Carry Law Booklet
      c) Original firearms training certificate
      d) Current photo identification (i.e. drivers license, state identification)
      e) 2” by 2” color head shot photo taken within the last thirty-days (i.e., passport type)
      f) If you have ever served in the military, you need to bring your DD214 Long Form (honorable discharge)
          with you.
      g) One of the following methods of payment for the NON-REFUNDABLE APPLICATION FEE:

                  1) personal check
                  2) money order
                  3) cashier's check
                  4) certified bank check
 
           in the following amounts made payable to the "Medina County Sheriff’s Office"

                  1)  $67.00 if Ohio Resident for 5 years or longer.

                  2)  $91.00 if Ohio resident less than 5 years.

                  WE WILL NOT ACCEPT CASH OR CREDIT CARDS

                  DO NOT BRING ANY FIREARMS TO THE SHERIFF'S OFFICE

 
6.  One applicant will be processed at their scheduled appointment time and their completed application, training
     certificate and if applicable, their DD214 Long Form will be reviewed for completeness and accuracy.  Fingerprints
     will be obtained through the Web Check System and sent electronically to the Bureau of Criminal Investigation &
     Identification for a background check.

7.  The applicant will provide all the necessary information to conduct the required background investigation by the
     Sheriff's Office. The applicant will then be notified, as outlined, of the next step on the process.

8.  The applicant will be contacted within forty-five days by phone when their permit is ready to be picked up.  If their
     application is denied, we will notify them by mail of the reason for denial.
